What to Expect After the Abortion Pill

The abortion pill involves two drugs taken in sequence: mifepristone, which stops the pregnancy from progressing, and misoprostol, which causes the uterus to contract and expel the pregnancy. Abdominal cramping and vaginal bleeding are expected. Women may also experience side effects like nausea, diarrhea, fatigue, and mild dizziness.

The key is knowing when symptoms cross from expected into something that needs attention.

Warning Signs That Require Emergency Care

Go to the nearest emergency room if you experience any of the following signs or symptoms:

  • Soaking through two or more pads per hour for two consecutive hours. This level of bleeding can indicate hemorrhage and should not be waited out.
  • Passing clots larger than a lemon. This is beyond what the process normally produces.
  • Fever of 100.4°F or higher lasting more than 24 hours, or a fever that begins several days after taking misoprostol. A delayed fever is often a sign of infection.
  • Severe pelvic or abdominal pain that worsens over time or does not respond to over-the-counter medication. Cramping should ease, not escalate.
  • Foul-smelling vaginal discharge. This may indicate a uterine infection.
  • Dizziness, fainting, or unusual weakness. These can signal significant blood loss.

If you go to the ER, tell the medical team what drugs you took and when. That information helps them treat you accurately.

Why Follow-Up Matters

Even without any of the above symptoms, a follow-up appointment after the abortion pill is an important step. It confirms whether the abortion was complete and rules out complications like retained tissue.
